Abstract

Over eighteen months Avanon reviewed 214 enterprise AI initiatives across Fortune 1000 companies — banking, healthcare, logistics, retail, industrial. Every project had a named executive sponsor, an approved budget of at least $250,000, and a defined business case at kickoff. Of those 214, 156 never reached production. That's a 73% pre-deployment failure rate. This paper examines the root causes and presents a framework for success.

1. Introduction

Ask a panel of CIOs why AI projects fail and you'll hear "the technology isn't ready" or "we don't have clean data." Both explanations are available. Neither is what the evidence shows. In our sample, 89% of failed projects had technically functional prototypes. In 71% of failures, the model hit or exceeded its accuracy target in offline evaluation. These were not projects killed by bad models. They were projects killed by organizational decisions after the model worked.

2. The Scope Creep Pattern

The leading cause — cited in 61% of post-mortems and independently corroborated by looking at timeline artifacts — was scope creep. Not scope creep in the "one more feature" sense. Scope creep in the structural sense: the project's definition of success silently expanded between kickoff and deployment, and no one renegotiated the budget, timeline, or risk envelope to match.

3. Ownership Ambiguity

In 48% of failed projects, at the time the project died, no single person could name the model's on-call owner. Engineering had handed it to a "data science platform team" that didn't exist. Product said it belonged to "whoever owns the CRM integration." Nobody had been given the incentive, the budget, or the calendar space to keep the thing alive.

This matters because enterprise AI is not a one-time deliverable. It decays. Schemas change, vendor APIs drift, error patterns shift. A model with no owner enters decay on day one and is uselessly wrong by day ninety.

4. The Shadow Period Problem

Successful projects in our sample ran the AI in shadow alongside humans for a median 47 days before cutting over. Failed projects either skipped the shadow (immediate production cutover, 24%) or ran an indefinite shadow that never ended (the model output was "available" but no one's workflow required consuming it, 19%). Indefinite shadow is worse than no shadow. It signals that the organization doesn't know what good looks like and isn't willing to commit to a definition.

5. The Framework That Works

Projects in the 27% that succeeded shared five operational traits:

A written and re-ratified scope memo at every phase gate — not a Jira epic, an actual memo signed by the sponsor, the owner, and the consumer.

A named single-threaded owner whose quarterly performance review includes the deployment.

A hard-edged shadow period with a pre-declared cutover date tied to specific accuracy and latency thresholds.

A procurement fast-track with pre-approved vendors so integration decisions take hours, not quarters.

A committed decommission path — what do we turn off the day this is in production?

214 enterprise AI projects across Fortune 1000 companies in banking, healthcare, logistics, retail, and industrial sectors.

Criteria

Named executive sponsor, minimum $250K approved budget, defined business case at kickoff.

Period

18-month observation window (October 2024 - April 2026).
